OSHA Recordkeeping

OSHA recordkeeping requirements can vary depending on the type of occupational health testing or workplace event involved. Employers should understand the difference between records maintained as part of an occupational health or compliance program and injuries or illnesses that must be entered on the OSHA 300 Log.

Work-related hearing loss meeting OSHA's recordability criteria may require entry on the OSHA 300 Log, while records such as respirator fit-testing results, respiratory medical evaluations, pulmonary function testing, and other occupational health documentation may be subject to separate OSHA record-retention requirements.

OSHA Compliance Forms & Employer Resources

Access commonly used occupational health forms, employee questionnaires, testing instructions, and workplace compliance resources for hearing conservation, respiratory protection, medical surveillance, vaccinations, and OSHA recordkeeping. These resources are provided to help employers prepare employees for on-site testing and maintain organized occupational health programs.
